Can I get the form for a missed final exam or Special Examination?

Yes. The Western SMC is commonly used to support Special Examination requests for missed finals. Submit the SMC together with your Academic Consideration Request through your faculty's academic counselling office. Can the doctor backdate the form?

Physicians can document a current medical assessment and the period of illness based on what you describe and clinical judgement. They cannot retroactively certify illness they did not observe.
