Informed Consent for Virtual Medical Consultation

This document outlines the informed consent for receiving healthcare services through MediNote's telehealth platform. Please read it carefully before your consultation. By proceeding with a virtual consultation through MediNote, you confirm that you have read, understood, and agree to the terms below.

Nature of Virtual Care

Your consultation will be conducted via telephone and/or secure messaging. Virtual care is a legitimate form of healthcare delivery recognized by provincial Colleges of Physicians and Surgeons across Canada. However, it has inherent limitations compared to in-person examinations.

By Proceeding, I Understand and Agree That:

  • 1.My consultation will be conducted via telephone and/or secure messaging technology.
  • 2.Virtual care has limitations and may not replace an in-person physical examination when one is clinically required.
  • 3.My physician may determine that my condition requires in-person assessment and may decline to provide the requested service.
  • 4.Technical failures may interrupt or delay my consultation.
  • 5.I am responsible for being in a private, secure location during my consultation.
  • 6.My personal health information will be collected, used, and stored as described in the Privacy Policy and Health Information Privacy Notice.
  • 7.I have the right to withdraw consent and end the consultation at any time.
  • 8.A physician-patient relationship will be established during this consultation.
  • 9.I am currently located in the province I selected during registration and confirm this is accurate.
  • 10.MediNote uses AI-assisted technology during the intake process to organize my health information. All clinical decisions are made by a licensed human physician. I may request a fully human-conducted intake by contacting support before my appointment.

Limitations of Virtual Care

I understand that virtual care is not appropriate for all medical conditions. In particular:

  • Emergency or life-threatening conditions require in-person emergency care (call 911)
  • Conditions requiring physical examination, diagnostic imaging, or laboratory testing may not be fully assessed virtually
  • The physician may recommend follow-up care, additional testing, or in-person evaluation when clinically indicated
  • MediNote is designed for episodic care and does not replace a primary care provider for ongoing health management

Clinical Discretion

The physician retains full clinical discretion regarding the appropriateness of any requested service. If the physician determines that a requested document (e.g., sick note, medical certificate) cannot be issued based on the clinical assessment, I understand that:

  • The physician is not obligated to issue the requested documentation
  • I will receive a full refund of the documentation fee in such cases
  • The physician may recommend alternative care pathways

Privacy and Confidentiality

I consent to the collection, use, and disclosure of my personal health information as necessary to provide healthcare services, as described in the Privacy Policy. I understand that:

  • All communications are encrypted and stored securely in compliance with PHIPA and PIPEDA
  • My medical records will be retained for a minimum of 10 years as required by law
  • I may request access to or correction of my health records at any time
  • I may withdraw my consent for future use of my information by contacting privacy@medinote.ca

Withdrawing Consent

I may withdraw my consent to virtual care at any time by ending the consultation or contacting MediNote support. Withdrawal of consent does not affect the legality of information collected prior to withdrawal. Certain records may be retained as required by law and professional regulatory obligations.
