What is the OSAP Disability Grant?
The OSAP Disability Grant umbrella covers two main funding streams for Ontario students with permanent disabilities, persistent or prolonged disabilities (PPD), or temporary disabilities: (1) the Bursary for Students with Disabilities (BSWD) — up to $20,000 per year for educational disability-related supports, and (2) the Canada Student Grant for Students with Disabilities (CSG-D) — up to $4,200 per year. Both grants require medical documentation from a licensed health professional confirming a disability.
What medical documentation does OSAP require?
OSAP requires a 'Disability Verification' form (sometimes called 'Documentation of Permanent Disability' or 'Documentation of Persistent or Prolonged Disability') signed by a licensed health-care professional — typically a physician, psychologist, or other regulated provider. The form confirms the student has a permanent, persistent or prolonged, or temporary disability that creates a functional limitation in pursuing post-secondary studies.
Can a MediNote physician complete the OSAP disability documentation?
Yes — for medical conditions that fall within the scope of family medicine. MediNote physicians are CPSO-licensed Ontario physicians and can complete OSAP's Disability Verification form for conditions such as ADHD (where previously diagnosed), chronic medical illnesses (Crohn's, lupus, diabetes complications), chronic pain, mood disorders, anxiety disorders, and other physician-diagnosable conditions. What is the difference between Permanent Disability and Persistent or Prolonged Disability for OSAP?
Permanent Disability is a condition that the student has had since childhood or that will continue indefinitely (lifelong). Persistent or Prolonged Disability (PPD) is a condition that has lasted, or is expected to last, for at least 12 months and impacts the student's ability to perform daily activities. The Disability Verification form has separate sections for each — your physician will check the box that matches your situation based on the medical assessment.
Will OSAP accept a form from a telehealth doctor?
Yes. OSAP accepts the Disability Verification form from any qualified, licensed health professional. The form requires the practitioner's name, license number, profession, and signature — not an in-person visit. MediNote physicians provide all required information on the form.
I already have a diagnosis from another doctor — can MediNote complete the form using that?
Yes. Bring any prior assessments, specialist letters, lab work, or psychiatric evaluations to your consultation. The MediNote physician can review existing documentation and complete the OSAP form based on the established diagnosis, avoiding duplicate assessments.
Can MediNote help with the OSAP application itself?
MediNote handles the medical documentation — the part that requires a physician. The OSAP application itself (financial, academic, identity sections) must be completed by the student through OSAP's online portal. After your consultation, upload the completed Disability Verification form to your OSAP application.
I have ADHD / anxiety / depression — is that enough for an OSAP disability grant?
Possibly. OSAP's disability grants are based on functional limitation, not diagnosis alone. The physician's role is to certify that the condition creates a functional barrier to your studies (e.g. concentration, attendance, energy, processing speed). If you have a diagnosed mental health condition that affects your studies, MediNote physicians can complete the appropriate OSAP form. For learning disabilities specifically, OSAP usually requires a psycho-educational assessment from a registered psychologist.
