Do I need to be in the same province as MediNote's physicians?

Can the doctor backdate my form?
Physicians can document a current medical assessment and the period of illness based on what you describe and clinical judgement. They cannot retroactively certify illness they did not observe. What if my university is not in your list?
Even if your specific university is not pre-loaded in MediNote, your physician can complete a generic Canadian medical certificate that includes all standard required elements (your name, dates affected, physician's name, license number, signature, contact information, statement of medical necessity). Most universities accept this when their official template is unavailable. Contact us before booking if you have a non-listed school and we'll confirm.
Will my specific diagnosis be on the form?
No. University medical certificates require the physician to confirm a medical illness or injury affected your ability to attend or complete academic work — but they do not require disclosure of your specific diagnosis. This protects your privacy under PHIPA and provincial privacy laws. The form simply confirms a medical condition was present without specifying what it was.
